Thank you for taking the time to share your experience and insights with us.  

Look Good Feel Better wants to understand what types of psychosocial support would be most helpful for people living with or beyond cancer across Canada. We want to learn about your experiences, needs, and preferences to help shape future programs and services. 

After submitting the survey, you can enter a draw to win one of eight $100-150 gifts. 

What we mean by psychosocial support 

Psychosocial support refers to non-medical support provided to those living with or affected by cancer to meet their physical, emotional, social, psychological, cultural, informational, spiritual, and practical needs throughout the cancer experience. 

Before you begin

This survey asks about your experiences with cancer and support services, including questions about challenges you may have faced. 

Please note:   

  • This survey is voluntary.  
  • Your responses are anonymous and confidential. 
  • The survey should take about 15 minutes to fill out.  
  • This survey is for those who are 18+ and have personal experience of living with cancer.   
  • By taking this survey, you agree to share your survey answers with LogicalOutcomes (a nonprofit consultancy facilitating this research), Look Good Feel Better, and the Psychosocial Oncology Lab at University of Ottawa.
  • There is a chance that anonymous quote(s) from your response will be shared in public-facing documents or reports. Comments that contain sensitive or identifying information will never be used.
